Adolescent Health Program Coordinator
Applications are being accepted for an Adolescent Health Program Coordinator. The Program Coordinator provides day-to-day program and staff supervision, monitors fidelity and quality of program delivery, and ensures data collection. The Program Coordinator works with community partners to recruit participants, schedules and conducts classes. This position serves as a resource to the schools and other organizations and groups on issues of teen health and connects youth and their families to the appropriate resources.
Permanency Navigator (Case Manager)
Applications are being accepted for a Case Manager to work directly with youth and young adults to help link them with community resources and services to successfully transition from homelessness to self-sufficiency. Working as a bridge/navigator between internal YST programs, outside community partners and housing options. Bachelor’s degree in Social Science field or an equivalent combination of education, training or experience is required for this position. Case management experience is preferred. Health Education Program Specialist
Applications are being accepted for a Health Education Specialist to provide health education, prevention information, and facilitates access to clinic visits, also work with expectant and parenting teens, provide resources and referrals for testing and treatment for at risk youth. Bachelor’s degree and experience with working with at risk youth is required (Master’s preferred). Some evening hours are required. Applications are accepted until position is filled. EOE
